Patients referred to the ADRC are asked to complete an application. An ADRC physician will review the application to determine whether an evaluation can be completed at the center. Applicants must be in good general health and fluent in English or Spanish. Our facility is generally unable to provide evaluations for people with a history of the following conditions:

• Neurological
• Medical
• Psychiatric

Neurological

Head injury (with post-traumatic cognitive/ neurological disorder)
Brain tumor
Encephalitis (with subsequent cognitive neurological disorder)
Seizure disorder (prior to dementia)
Parkinsons disease
Sensory loss (severe enough to invalidate testing)

Medical

Cancer (within the last 5 years, except non-melonoma skin cancers)
Heart Attack (within 6 months)
Bypass Operation (if cognitive decline appears secondary to surgery)
Open heart surgery (if cognitive decline appears secondary to surgery)
Thyroid disease (current, if not stable on medication)
Diabetes requiring insulin
Renal disease requiring dialysis

Psychiatric

Hospitalization for psychiatric disorder
Schizophrenia
Major depression
Alcohol/Drug Abuse

ADRC staff work closely with referring health care providers to ensure the best care possible for their patients. At the patient’s request, evaluation results will be sent to referring physicians.
